This policy explains how patient-submitted information may be shared.

1. Sharing with the chosen clinician

Patient-submitted information is shared with the chosen independent clinician to support the consultation request and consultation.

The chosen clinician may use relevant information to decide whether more information, GP records, physical health checks, urgent support or alternative care is appropriate.
